The PA Independent Enrollment Broker– Maximus- will be on site from 1:00-3:00 PM in the Wade Blank Rooms to present on the following information:

• Community Health Choices (CHC), who is covered and the MCO selection process
• CHC Home and Community Based Services enrollment process and how eligibility criteria differs from the old fee for service waivers
• The continuity of care period for those who had Long Term services prior to 1/1/2019
• Review the steps to apply and the best ways to begin the waiver application process
• The process for someone in a facility who wants to transition back into the community
