Job Description:
This position provides routine ongoing, preventive and minor maintenance for the 714 Sovereign and Liberty Resources, Inc. facility when needed.

Required Education and Experience:
High School Diploma or equivalent and a minimum of three years related maintenance experience in commercial buildings. Experience in plumbing, HVAC and electrical skills, including troubleshooting, repair and installation are a plus. Certifications from technical/trade schools are preferred. Must have the ability to work independently or in a team and have good communication skills.


Supports Coordinator – ACP

Job Description:

This position provides direct services to consumers.  It ensures consumers receive the services needed to live, learn, work and socialize in their communities.   This includes the ability to identify, provide and maintain regular supports to assure continuity of care while maintaining compliance with all contract and funding regulations.

Required Education and Experience:

Bachelor’s Degree in social work, social science or related field of human service, such as psychology and one year of case management experience or at least six months as a Home and Community Based Services waiver/program participant;

Or

Associates Degree in social work, social science or related field of human services, such as psychology and two years of case management experience or at least one year of professional experience and at least on year as a Home and Community Based Services waiver/program participant;

Or

12 credit hours of human services course work from an accredited college or university and at least four years of professional experience and at least two years of experience and at least two years as a Home and Community Based Services waiver/program participant.

Incumbent must have required training, including at a minimum: Office of Long Term Living (OLTL) Service Coordination Training.  Each Supports Coordinator will be required to have 40 hours of training during the first year of employment and 20 hours annually.  Supports Coordinators working with individuals with traumatic brain injury must also have at least one year of experience in rehabilitation and/or working with individuals with traumatic brain injury.
